Talk of treaty ban on mercury concerns scientists

LONDON (AP) — Scientists are warning officials negotiating a global treaty on mercury that banning the deadly chemical completely would be dangerous for public health because of the chemical's use in vaccines.

The ban option is one of several proposals on the table for a meeting later this month in Nairobi, but a final treaty isn't expected until 2013.

According to the World Health Organization, mercury is one of the top 10 chemicals of public health concern and is highly toxic. Most of the worry is centered on mercury emissions from burning coal, gold mining and people eating mercury-tainted fish.

Mercury in small amounts is also found in many products including light bulbs, batteries and thermometers. WHO advises such products to be phased out, suggesting for example, that health systems switch to digital thermometers instead.

The problem is that a proposed ban might include thiomersal, also known as thimerosal, a mercury compound used to prevent contamination and extend the shelf life of vaccines, many scientists say. It is used in about 300 million shots worldwide, against diseases including flu, tetanus, hepatitis B, diptheria and meningitis.

"Not being able to use mercury is not a viable option," said David Wood, a WHO vaccines expert.

Wood said there isn't a viable alternative to thiomersal at the moment. If banned, pharmaceuticals would likely have to switch to preservative-free vaccines, which would complicate the supply chain and vaccination campaigns in poor countries, since the injections would have a much shorter shelf life. Costs would also spike since manufacturers would need to reconfigure their factories.

In 2009, the United Nations Environment Programme, or UNEP, began working on a legally binding global treaty on mercury. At the end of October, the third of five meetings to hammer out a treaty will take place in Nairobi.

"The document is a draft at the moment, so some of these proposals have to be taken with a grain of salt," said Tim Kasten, head of the chemicals branch at UNEP. Kasten said the amount of mercury in vaccines is so minute it doesn't threaten the environment. He said there could be provisions to allow mercury for certain uses, such as in dental fillings and vaccines.

But according to an annex in the draft document, there is currently no "allowable use exemption" for mercury products in pharmaceutical products, putting vaccines in the same category as banned mercury-containing paints and pesticides.

"That would be a terrible idea," said Paul Offit, an infectious diseases expert at the University of Pennsylvania. "It would be another tragic example of us not being able to explain to the public where the real risk lies."

Thiomersal has mostly been removed from childhood vaccines in the U.S. and Canada. In some European countries, including Norway and Sweden, manufacturers have been encouraged to make thiomersal-free vaccines — and no other uses of mercury as a medical preservative are allowed.

Fears about thiomersal in vaccines were raised after a flawed medical study in 1998 linked a common childhood injection to autism. Even though that study didn't involve mercury, it ignited fears about vaccines in general, including those containing thiomersal.

Concerns were also heightened after the U.S. decided to largely remove thiomersal from many injections as a precautionary measure in the 1990s. Numerous studies have found no sign the mercury compound is risky but many anti-vaccine groups still believe it may be linked to autism and other health problems.

Experts hope countries won't go overboard in their attempts to control the substance.

"Provided you know the risks and it's handled properly, there isn't a problem," said Andrew Nelson, a toxicology expert at the University of Leeds. "The health of so many millions of children benefit from vaccines containing mercury that an absolute ban is ridiculous."

There's alot of talk of having to reconfigure factories and vaccine shelf life in poor countries.. but they would not have taken the precaution of removing it from some vaccines in the west if there was no risk. Drug companies are making billions in vaccination programs the onus should be on them to adapt to changing laws.

A nurse injects a patient with a syringe of antibiotics, reloads and moves on to the next patient in line. The syringe isn't sterilized, the needle is not replaced, and the patient is at risk of contracting a disease from the very shot that is supposed to cure him.

It's a disturbing scenario, and one that plays out each day in many poor countries. About 40 percent of all injections are given with unsterilized, reused syringes and needles, reports the World Health Organization. An estimated 1.3 million deaths -- and 21.7 million new Hepatitis B infections -- occur each year as a result of the unsafe practice.

To prevent this from happening, organizations like UNICEF and the WHO have moved to syringes that "self destruct" after one use to protect patients from contracting blood-borne diseases. The devices, called auto-disable syringes, jam up after one injection and can't be reused. The casing is engineered to break if someone tries to forcefully refill it.

So how many diseases have been prevented and how many have been spread in regions where diseases like hepatitis B and HIV are in higher rates anyway? Why is it that it is only in this year that they are coming up with single shot syringes when sharing needles is KNOWN to spread disease so easily?

A burgeoning trade in used medical equipment has caused a deadly outbreak of hepatitis B in India, highlighting a major problem for the health-care sector. Kristin Elisabeth Solberg reports.
An investigation into a large outbreak of hepatitis B in India this month has uncovered a deadly trade in used medical equipment, and highlighted corruption and ignorance in the health-care sector. But in a country where unsafe injection practices flourish, experts say the outbreak, which has claimed at least 60 lives, is merely a symptom of a much bigger problem.
Alarm bells went off when at least 240 people fell ill in a district of the wealthy Indian state of Gujarat last month. Health officials there traced the source of the infections back to tainted needles. The following probe not only unearthed a network trading illegally in medical waste but also found that doctors were recycling their equipment despite the low cost of disposable syringes.
Police and health authorities discovered warehouses filled with recycled medical waste, waiting to be sold on the black market. Needles and syringes, which should have been disposed of, were instead collected from health facilities by waste-pickers. They were then allegedly cleaned, repackaged, and resold to private medical clinics.

Raiding clinics, police, and health officials found that not only unlicensed medical practitioners but also fully trained doctors administered injections with recycled equipment. “All across the state, we found 349 quacks and 8 to 9 doctors who were not following safe injections practices”, says Amarjit Singh, Gujarat's health commissioner.
Two doctors, a father and son, believed to have transmitted hepatitis B using the same syringe on several patients, have been charged with culpable homicide, and could face years in prison. A handful of other medical practitioners have also been put behind bars, and about two dozen medical-scrap dealers are under police investigation. Authorities have also seized several hundred tonnes of medical waste, and temporarily shut down nine clinics.
The scandal in Gujarat has shocked many both within and outside India, but the recycling of medical equipment is an old problem on the subcontinent. Out of ignorance, lack of training, or simply to cut costs, doctors all over the country reuse medical equipment. A large 2004 study by the Indian Clinical Epidemiology Network found that more than 30% of the 3—6 billion injections administered each year were done with used equipment. The study also revealed that in almost 10% of health facilities nationwide, used syringes and needles were sold off to waste-pickers.
Since then, the Indian Government has introduced the compulsory use of auto-disposable syringes in the public sector. Although this may have slightly reduced the number of unsafe injections in the past few years, the problem is still large, according to Narenda K Arora, executive director of the International Clinical Epidemiology Network, who led the 2004 study. Arora says that he is not surprised by the virus outbreak in Gujarat. “This incident should be used to take accelerated action. Because this is happening all the time”, he says.
According to Arora, several measures need to be introduced to combat the problem. These should include tighter control of equipment to make sure it cannot be reused; better waste management to ensure that medical waste is not recycled for its original purpose; compulsory training of health personnel in how to administer injections; and awareness raising to inform patients about alternatives to injections, as well as how to identify and demand the use of new syringes.
No one can tell how many tainted syringes are still in the black market, but state authorities in Gujarat say they are “confident” that they have taken the necessary steps to prevent a second outbreak of hepatitis B there. They have launched a large-scale vaccination programme in the affected areas, and identified and treated people with symptoms. Moreover, health commissioner Singh says, with the arrest of some senior doctors, “the message” has got through. The state government is also drafting a bill which calls for the compulsory registration of all doctors. This would include regular inspections to ensure that clinics meet safety standards. But for now, in the absence of safe-injection practices and in the presence of greed and ignorance, new infections and more deaths are likely in India.

Again, I still feel that the vaccines themselves are not to blame, but rather the problem is that they are being developed and administered through corrupt state agencies and profit-motivated corporations.

There is a fundamental dilemma when you are trying to use a capitalist instrument to deliver public goods.

What alternatives are there for obtaining needed vaccines without going through the big pharmaceutical giants?

There were vaccines before big pharma and not all act in that manner.. and there is the public sector. You speak as though we are at their mercy. The whole point of WL is exposing corruption and for free expression.. and (reiterating yet again) I am not blaming the concept of vaccines, I have never said I am against them. I am posting facts and trying to get the word out through yet another media blackout. The media has gone out of it's way to convince people that they have to either accept potentially risky vaccines or have horrible pandemics. The "oh nos without vaccines we'd all be dead!" fear mongering is being used as an excuse to turn entire populations into dispensable lab rats.. and when vaccines are being given with used syringes a better "alternative" would be not to have them at all. They had been vaccinating AND infecting people in the same shot.

It is not charity or "helping humanity" either it is criminal abuse and they get away with it because:
1. They are working in places where patients have no rights or options when something goes wrong.
2. They've been moral free pass like the christian missionaries of old.. and if you dare to question guess what? You get accused of risking the lives of all those little babies that might catch measles which just deflects the subject away from corporate accountability.

A guard dog can offer fantastic personal protection.. but not if it is rabid.  It needs to be either put down or put on a leash.

It WAS on a leash but the US government made it illegal to sue vaccine companies..

Considering the population of Canada is ~34.7 million, that'd probably float somewhere around 150 people infected if the whole country is treated in these facilities once (having one or the other of the diseases, or both). In my opinion, since we have the technology and the expertise it should never happen, but considering rates of infection in other countries I think what we got is acceptable by comparison.

Sort of makes sense, I guess, to not be able so sue vaccine companies. As in, you can't sue McDonald's for making you fat - it's sort of an accepted side effect. If you have a reaction to the vaccine and you go blind or something then I can see suing might hold up. The government is probably supposed to give you a care package or whatever. Basket of fruit, knowing Harper :P
What I'm really getting at is no company should be immune to legal reprimand, but it doesn't make sense to have "migraines" as a known and published side effect by that company, and then that company gets sued because some girl got migraines from whatever drug it is. I'd consider it like an "accepted risk" dealio.

What disturbed me was a while back when our health department sent a bundle of body bags along with the H1N1 vaccine to the aboriginal settlements. Downright disrespectful - it's like saying "we're leaving you to die."

GlaxoSmithKline Argentina Laboratories company was fined 400,000 pesos by Judge Marcelo Aguinsky following a report issued by the National Administration of Medicine, Food and Technology (ANMAT in Spanish) for the killing of 14 babies during illegal lab vaccine trials conducted between 2007 and 2008.

Likewise, two doctors -Héctor Abate, and Miguel Tregnaghi- were fined with 300,000 pesos each for irregularities during the studies.

The charges included experimenting with human beings, falsifying parental authorizations so babies could participate in vaccine-trials conducted by the laboratory from 2007 to 2008.

Since 2007, 15,000 children under the age of one from Mendoza, San Juan and Santiago del Estero have been included in the research protocol, a statement of what the study is trying to achieve. Babies were recruited from poor families that attended to public hospitals.


A total of 7 babies died in Santiago del Estero; 5 in Mendoza; and 2 in San Juan.

Pediatrician Ana Marchese, who reported the case through the Argentine Federation of Health Professionals (FESPROSA in Spanish), and was working at the Eva Perón children's public hospital in Santiago del Estero when the studies wee being conducted, said this morning in conversations with Continental AM radio that “GSK Argentina set an protocol at the hospital, and recruited several doctors working there.”

“These doctors took advantage of many illiterate parents whom take their children for treatment by pressuring and forcing them into signing these 28-page consent forms and getting them involved in the trials.”   

“Laboratories can't experiment in Europe or the United States, so they come to do it in third-world countries."

Colombian and Panama were also chosen by GSK as staging grounds for trials of the vaccine against the pneumococcal bacteria.

Likewise, Marchese, explained the modus operandi: “Once a picked patient arrived, it would automatically disappear to be taken somewhere else in order to be treated by those doctors specially recruited by GSK. These kind of practices are not legal and occurred without any type of state control, plus they don’t comply with minimum ethical requirements.”

Marchese also remembered that “laboratory trials on human beings are not legalized in Argentina.”

Furthermore, the pediatrician explained that “it is also known that in various particular cases, the doctors who had conducted the trials did not answer the calls made by the worried parents after witnessing their babies’ reactions to the vaccines.”   

According to Marchese, “there already exist very good vaccines for the same diseases, but we all know how laboratories work, they only care for their own business.”

To end, Doctor Marchese aimed to Santiago del Estero Governor, Gerardo Zamora, who “never ever came out to stage to comment on the case, and same happened with national deputies and senators that didn’t even bother into discussing a hot topic that was echoed worldwide. I’m also ashamed of the scientific community that also kept its mouth shut.”       

Julieta Ovejero, great aunt of one of the six babies who died in Santiago del Estero, said that “A lot of people wanted to leave the protocol but they weren’t allowed; they forced them to continue under the threat that if they leave they won't receive any other vaccine.”

During 2008, the vaccine trial was still ongoing despite the reports issued by FESPROSA, and those in charge of the study told reporters that the procedures were being carried out in a lawful manner.

On the contrary, the ruling states that the laboratory as well the involved doctors broke all legal requirements for conducting clinical tests on babies.   

Surprisingly, during same year pediatrician Enrique Smith, one of the lead investigators told reporters: “Only 12 have died throughout the country, which is a very low figure if we compare it with the deaths produced by respiratory illnesses caused by the pneumococcal bacteria.”

In Santiago del Estero, one of the country's poorest provinces, the trials were authorized when Enrique's brother, Juan Carlos Smith, was provincial health minister.

According to Fesprosa, “the laboratory paid $8,000 pesos for each child included in the study, but none (of that money) remained in the province that lends the public facilities and the health personnel for the private research.”

GlaxoSmithKline is a global pharmaceutical, biologics, vaccines and consumer healthcare company headquartered in London, United Kingdom. It is the world's third-largest pharmaceutical company measured by revenues after Johnson & Johnson and Pfizer.

Ironically, if one visits GlaxoSmithKline Argentina web site it welcomes the reader with a company disclosure that says: “We have a challenging and inspiring mission to improve the quality of human life by enabling people to do more, feel better and live longer."

According to www.GSK.com site, Glaxo was originally a baby food manufacturer processing local milk into a baby food by the same name, and founded in Bunnythorpe, New Zealand in 1904. The product was sold in the 1930s under the slogan "Glaxo builds bonny babies".

GlaxoSmithKline Argentina manufacturing 28,333 m2 centre is located in Northern Greater Buenos Aires town of San Fernando.[/ex]

This has become a trend.. even a matter of policy. Preying on illiterate parents.. and one the lead investigators called these deaths small compared to the diseases they might prevent.. even though it was an illegal trial! This disgusting criminal activity has GOT to stop. It's not like the banks.. they just take people's homes away.. these bastards are actually using people as lab rats. A resource for experimentation.. it seems the fact that some of their victims cannot read or write enough to consent makes them worth less than the educated. Poor people should not be seen as fair game. This is the same as slavery.. using people as pieces of meat to make a dollar.

..and no they didn't get thrown in prison. They copped a fine. A BILLION dollar company got a small fine (compared to how much they make). Why do the doctors involved not get a prison term? Some parents tried to exit the trial but weren't able. This is MURDER to me and society needs to see it as such.

Lives should NOT be seen as expendable "for the greater good".

Yes, the OPV theory (oral Polio vaccine) has been suppressed by the 'scientific community' in favor of a theory that points to humans who eat monkeys. Their argument is that in cleaning and preparing the monkey meat for consumption, humans came into contact with the simian version of HIV, and that's where the virus crossed over to humans.

Bollocks, if you ask me. The 'scientific community' can't face the prospect that in the effort to eradicate polio, we created an HIV pandemic in Africa. Much easier to blame the 'monkey-eating natives', than the medical establishment.

It's particularly brutal how the chimpanzees were treated - organs remove without anaesthetic, only a sedative to induce paralysis. And those surgeries were conducted in front of the other chimps who would lament and mourn for their fellow captives. UNCONSCIONABLE
